1password8 Safari 15.1 won't login

Hi 1Password folks,
I'm having a random problem, where I start safari and won't be prompted for my password. Oddly the 1password app might already be logged in or that will work, but not safari.
I've done a few reinstalls, but no joy.
It's also not consistent, as I can ignore, come back to try again later, and it will be signed in on safari.
