Auto locks despite settings otherwise

I've set auto-lock on exit to never, and disabled auto-lock on device lock. However, I find that I have to authenticate into 1Pass pretty much every time I do anything with it, even if I've not locked my phone.

I've seen similar threads that haven't resulted in any good answers, partially cause the authors of those posts never responded to request for more information. I'm hoping this one will have some better luck, since I plan to respond!

What do you need from me to help troubleshoot this?
