I have the same problem, though now for me with the latest version of 1Password for Safari it won’t autofill even if the 1Password app is unlocked - and reported it last week. No response yet from 1Password Support.

Found a solution for me which may also work for you. See:

I am not in the fill stage.. The browser extension is not able to open and unlock the 1password mac app. The browser extension works fine while the mac app is open and logged in, but if the mac app is not open or not logged in the browser opens the login popup modal, then after entering the password it unlocks the browser extension briefly for few milliseconds and locks back again. The mac app is never unlocked due to this action no matter how many times I enter the password in that prompt initiated from the browser addon. Somthing feels like the browser extension is not able to send signal to open and unlock the mac app.

So you’re getting the 1Password prompt in-line in the website’s username and password fields, but it won’t unlock? Check the 1Password for Safari General preferences. Does it show that the Integration Status is “Connected”? If not, that’s at least a symptom of the problem. You might try toggling the setting of “Connect with 1Password in the browser” in the 1Password → Browser preferences and see if that helps. You may have to quit 1Password completely - use the Quit command in the menu bar item - to get it to “take”.

If that doesn’t help, you’ll probably need to get further assistance from 1Password Support.

Thanks for the reply. This is expected behaviour currently. You’ll need to keep 1Password’s icon in the menu bar for 1Password to be able to continue running in the background if you close the main window.

We have an issue open to improve how the desktop app is launched by the browser extension, if it isn’t already running, and hopefully improvements can be made in future versions of 1Password. For the time being, I recommend keeping the menu bar icon enabled.
