Copy and paste password fails frequently, kills input on destination window

Often if I go to copy and past an email address, password or OTP when I go to past into the destination window (Safari, Chrome, etc) the paste won't work (I get beep) and then the destination window will not accept input. Sometimes the window is locked out and I have to quick 1P 8 and then the destination window will often have some or all of what I was pasting in the field (sometimes), but usually becomes responsive again.
