Having 1Password save 3 items for a sign in to a website?

I have a website that requires 3 bits of information for a sign in (Company ID, User ID, and Password).  I am able to get 1Password to save the User ID and the Password, but can't figure out how to get it to save the Company ID, and then to fill in all three items when I go to the website to sign in.   I am doing the sign in to the website from a Windows 11 computer.
