I can't create new login in 1password on my Mac

Hi team,
I'm facing a problem with 1password on my Mac when I try to create a new login. I try to create a new login on Chrome extension and it works and syncs to 1password on my iPhone successfully.

1Password Version: Mac: 8.9.4
Extension Version: 2.3.8
OS Version: MacOS 12.5.1
Browser:_ Chrome
