Setting up Universal Autofill

I am running Tahoe 28.1 and 1Password for Mac 8.11.12

I have tried following the instruction to set up Universal Autofill but 1Password does not appear under Accessibility and neither done a + button got add it.

Is this a 1Password or a macOS issue?

Thanks for posting those screenshots! It looks like you're looking at Accessibility settings in general and not Accessibility under Privacy & Security. Try these steps:

Click on the Apple logo  in the menu bar then click System Settings.
Scroll down and click Privacy & Security in the sidebar (not Accessibility).
On the page that opens, click Accessibility in the list.
Turn on the toggle beside 1Password, then enter the password you use to log in to your Mac.
- If you don't see 1Password, click the + button, enter the password you use to log in to your Mac, and locate the 1Password app in your Applications folder, then select it and click Open.

Let me know if that doesn't work.

I'm sorry that you're running into trouble when setting up Universal Autofill. Can you post a screenshot of the Accessibility settings window from macOS Settings so that I can better understand the situation?
