I've got this problem too. There was another thread related to this but it was closed without any confirmation of a known problem or a resolution. Very frustrating.
I may have found the issue... I'm changing me post now before checking... updating 1Password doesn't update the extension in Safari. Updates to 1Password would do well to advise users to do so.
I think I’ve got it working. Again, updating 1Password doesn't update the extension in Safari. I have since updated the extension and it seems to have solved the issue.
When updating 1Password it would be helpful to be told an extension also needs updating. As an example Audio Hijack is good at doing that.

1Password for Safari is a separate app from the 1Password desktop app and will update separately. However, you can set the Mac App Store to automatically update 1Password for Safari:
