The best item type for this would likely be an Identity item. While I can't say for certain that 1Password will save and fill every field on every form, you can try the following steps with the 1Password browser extension:

1. Open the page with the form in question.
2. Type in or fill your information but don't submit the form yet.
3. Right click the 1Password icon in your browser's toolbar, then click Save > Identity.
4. Choose New Item, then click Save.

Hi @bobarnett, thanks for following up with me. I'm sorry to hear it only saved some of your information, and that the extension and Windows app may not be syncing. 

Is this form on a website that's publicly accessible? If so, we'd be happy to take a look to see if there are any better options for setting up an item to fill as expected. 

Also, are you now seeing the identity in 1Password for Windows, or does it appear that it's not syncing as expected?
