When I use right-click -> "Duplicate..." , I get two items with the same name.

How can I tell which one is the "original" and which one is the duplicate.

I do not want to modify the original because I have references to it via it's Secret Reference in scripts via

$(op read "xxxx")

It's there any way to tell from the UI which one is the original and which the duplicate.

I was expecting to see something in the history area like of each item something like

Hi @ecerulm,

Good catch! If the original items had previous versions, the duplicate (new) item will not have those. But otherwise, you're right there's no way to distinguish them. Your use case makes sense, though, so I've submitted this as a feature request to be considered.
