SSH error: signing failed for RSA "" from agent: agent refused operation

Hey, I'm having a problem with the ssh agent. Any time I'm trying to use ssh and authenticate (for multiple servers), I'm getting the following error:

sign_and_send_pubkey: signing failed for RSA "" from agent: agent refused operation
Received disconnect from 1.2.3.4 port 22:2: Too many authentication failures
Disconnected from 1.2.3.4 port 22

Additionally, the 1Password app shows an icon in the top right corner with a locked status, even though it is not locked and I'm able to access all passwords, etc.

Using 1Password for Mac 8.10.64 (81064007).
