Since the last update of the 1Password app on my iPhone and the corresponding update of the Safari extension, I have see this error at least once a day. I am using 1Password version 8.12.24.

I tried clearing cache items related to 1Password and I tried reinstalling the app.

Hello @kwedinger! 👋

I'm sorry that you're seeing an unexpected "Something went wrong" message when using 1Password for Safari. Are you using lockdown mode on your device by any chance?

I look forward to hearing from you.

I’ve same issue.

iOS 26.5.2 (same problem with 26.5.1)

1Password extension 8.12.26 (same result with prior version, 8.12.24 i believe)

I do have lockdown turned on.

Hi @sia, thanks for posting!

Lockdown Mode isn't supported when using the 1Password for Safari browser extension, as it disables some of the core browser functionality that the extension needs in order to function properly.

With that said, the repeating database error is a more recent behaviour that might be due to changes in either 1Password or iOS, and our development team is investigating. I’ve added your case to the internal issue.

In the meantime, using native AutoFill to fill and save passwords with 1Password will be your best bet while your device is in Lockdown Mode.

I’ve being running two phones in lockdown mode for nearly a year, with extension enabled and without a (visible) issue; I may not noticed that 1Password extension was not working properly since i also have AutoFill enabled. But with current version of extension + 1Password app that database failure screen happens on most of web  pages in Safari.

I’ve checked an the older phone running iOS 18.7.8 (also in lockdown mode) and it has same issue now; so it’s not interaction with newer iOS, but just a problem with recent version of 1Password+extension and probably lockdown mode; though it seems original reporter did not have it enabled.

I’d keep tracking this thread. Thanks!

I have also started receiving this popup on my iPhone. I’m not using Lockdown mode.
