Passkey login on Safari/MacOS rarely works

Trying to log in to work SSO on Safari/MacOS, the passkey login rarely appears. I have to reload 5-10 times before 1Password finally shows passkey login (and sometimes it just doesn’t work). I have 1Password app and Safari extension unlocked. These issues seem to have started within past few weeks/last month, either due to MacOS version upgrade or 1P update (or both).

Most of the time, it just displays the Safari-native passkey dialog. Other times, nothing happens (neither Safari-native nor 1Password login). Eventually (hopefully), 1Password will finally “click” and show the passkey login.

I do not have these issues with same website on Firefox or Chrome -- they show 1P passkey login immediately. In Safari extension, I have tried with “universal login” enabled & disabled, with no change. I did not enable “MacOS Autofill”, as that was a worse experience overall.

Unsure if related, but Safari settings > Extensions shows this error.
