1Pass for Safari clueless while menubar 1Pass works fine

With 1Pass 8 and 1Pass for Safari I continue to have problems as shown in the attachment. I will attempt to fill in a name/pw field and 1Pass doesn't appear. If I go to the menubar I immediately get ...

The 1PW Safari browser app needs work. It continues to have multiple issues.
