It sounds like you're both running into a known issue where 1Password's suggestion menu will stop working and 1Password won't fill login credentials. Our developers have identified an issue in Safari 18.4 that is involved with this behavior and we're working with our friends at Apple to get it resolved.
For the time being, opening the website in a new tab should get things working again. If that doesn't work then restart Safari by completely closing it and then reopening it.
Since we have an existing thread here, I'll close this thread as a duplicate.

Hi Dave, this morning, the banking tab did not fill with "1Password for Safari". The tab was left overnight in a Safari window with a number of other tabs/websites that I use regularly. I opened the same login page in another tab, and in this new tab "1Password for Safari" did work OK. I now have ONE Safari window with 14 tabs, among them two for the same banking page, with "1Password for Safari" behaving differently in these two tabs. See screenshots. Crazy!
BTW: Autofill (using the 1P icon from the Safari Toolbar) ONLY works in the 2nd and new tab, it fails to fill in the first tab - the one with the black "Open" on the "1Password for Safari" icon.

Have any resolutions been found to fix this issue?
I am still experiencing the autofill selection not appearing. The only way I can consistently get it to function again is to quit Safari and launch again. Then it will work for a while.
One thing I can tell you is that when the issue occurs if I click on the 1PW icon in the Toolbar, the window that opens display a window with only a progress wheel in the center for several seconds while it loads the information for the site I am on. Then I get the standard window with the login information so I can copy and paste.

Just an update.
Apple just rolled an update to macOS Sequoia 15.5. That came with an update to Safari 18.5 (20621.2.5.11.8).
It is still having the issue of the autofill selection not appearing, intermittently, at the user/PW field.
BUT, when I click on the 1PW icon in the menu bar, I no longer get the progress wheel, that loads for 4-5 seconds, in the middle of the window that opens. It immediately loads the information window that is expected from 1PW.
I attached a pic to make sure you know what I am talking about =)
Thought the team might be able to pull info from the way Safari is acting.
