I did some further testing on my end, and the additional fields you have in the Login item aren't causing the issue. From my testing, it looks like the username field is being auto-filled in the first box. Here's a screenshot for reference on how I found that:

Updating the existing Login item on that page is changing the username instead of creating an additional field.

Could you create another custom field labelled "username", enter your username below it and then delete the top "username" field? Here's what that looks like:
