How to save custom forms?

I live in Canada, and Cortico is one of the largest tools for health care appointments. I would love to be able to save the data from my whole family to fill that form. You can see the form here...

Thank you for bringing this to our attention, it seems 1Password is having a hard time detecting the fields on this page. I have filed an issue with our development team to see if we can improve the behaviour on this page in a future update.
