I just found the new menu option under Help -> Troubleshooting -> Fix Missing Safari Extension.

I have this same problem. The extension was installed, and working. Then, this morning, the window wouldn't open when clicking on the icon in the toolbar, so I restarted Safari.
When it restarted, the icon was gone, and the extension was missing from the list in preferences.
I uninstalled the "1Password for Safari" app, reinstalled it, but nothing changed.
