Hey @dcblack56! We don't yet have a way to report filling issues via the extension. However, I can leave feedback about that to our dev team on your behalf.
In the meantime, if you run into any sort of trouble, you can reach out to us here (or via email at mailto:support@1Password.com). The details that you've shared so far have been great. However, I was unable to find the form from your recording at Paddle.com. Could you let me know the exact URL to the form you're having trouble with?
