This used to happen in macOS 12 Sierra, but doesn't seem to have happened since then in macOS 13 Ventura, macOS 14 Mojave, or macOS 15 Sequoia.
It'll be worth updating to a version of macOS actively supported by Apple if your Mac is capable: ☞ Update macOS on Mac - Apple Support
In the meantime, fully quit Safari (right-click it in the Dock > Quit) then open and unlock the main 1Password app, and click the Help menu > Troubleshooting > Fix Missing Safari Extension. That should then bring it back in Safari when you next open it.
