Different one time password between iOS and Windoes
I have few accounts(more than 1) set with one time password, I just found it few months ago that if I log into these account with one time password generated by chrome extension on windows, I couldn't log in. But if I check the one time password on my iPhone and that password could let me log in.
Than I took more attention on this and found that the one time password for same account are different between ios and windows at same time.
For example, below are one time password for one account generated by ios and windows at same time:
ios: 252131
windows: 196064
1Password Version: 1Password for Windows 8.10.56 (81056028)
Extension Version: 8.10.56.28
OS Version: 10.0.22631.4602

I'd be 99.9% sure that your system clocks are out of sync by at least 30 seconds. 1Password uses the device's clock to generate one-time passwords.
Go here on each device to see how close to the actual time they are: https://time.is/
Adjust both devices so that they're exactly synced with internet time, and ideally turn on automatic syncing of the time in your devices' settings.

I'd be 99.9% sure that you are correct! I just check the time and found last sync of my computer was Oct 24, 2024 which was close to the first time I noticed the problem, and weird thing is the automatic sync up is enabled on my computer, that's why I had never thought about it even I also doubted it's caused by clock/timezones.
