If I have a file attached to an item, it is curly not possible to rename the attachment. To achieve that I have to download the attachment to a device, rename it, attach it to the item and delete the old attachment.
Would be nice to have such a possibility directly in the app.

You're right that, to rename an existing attachment, you'll need to download the attachment to your iPhone or iPad, rename it using the Files app, and then add it back to the appropriate item in 1Password.
