Password links I delete keep reappearing. Why?

To delete an item, right click on it in the item list, and then select delete:

If you want to prevent any possibility of recovering that item once deleted:

1. Sign in to your account at https://start.1password.com/
2. Open the vault that contained the item.
3. Click View Recently Deleted in the sidebar.
4. Click Destroy next to the item.

I have a password item for PhotoWoa. _It has my log in name, my password, and the internet address. When I delete it because I have two of the same file it later reappears in my account list. Why? How do I permeantly get rid of the duplicate file?
