Safari Extension In-Page Suggestions Missing

This was resolved by enabling the "Intersection Observer" feature in Safari.

Open the Settings app from the home screen
Tap Safari
Scroll to the bottom and tap Advanced
Tap Experimental Features
Turn on Intersection Observer
