Thank you for confirming that you can still reproduce the issue. It looks like the issue doesn't occur consistently for all users, another colleague of mine is able to consistently reproduce the issue while I can only reproduce it if I use multiple accounts. My colleague can reproduce the issue even with a single account.
I've added this information to the internal work item for the issue so that our development team is aware and can investigate further.

I think I see why some say it is focusing correctly, and others say it is not: That is because the FIRST time 1Password icon in Safari's toolbar is clicked, the focus is correct. Subsequent clicks of the icon to bring up 1Password and the focus is on the Vault (incorrect). So if you are just starting Safari and first-time click on the 1Password icon, you will think it is operating correctly because the focus is correct then. But subsequent clicks on the icon will not direct the focus properly. Does this help?

I'm no longer able to reproduce the issue using the latest version of macOS and 1Password for Safari. When I click on the 1Password icon in Safari's toolbar, the search field is focused automatically.
Are you still able to reproduce the issue? If you are then which version of macOS and 1Password for Safari are you using?
