Scanning QR code Fails

I'm trying to active 2FA from Cisco Duo. I think it's an iframe in the browser. I've tried everything in the app to scan the QR code and it doesn't detect it. This isn't the first time. I'm wondering...

This might actually not be a 2FA QR code, so maybe that's why it's failing. If I try to capture it on the 1Password app via my phone camera it also doesn't work.
