It seems strange that there are 6 items saved for this one site. Can you let me know if the username is the same on all of them as well as the password?
If you go to smile.amazon.com and try to sign in are you seeing a 'Save in 1Password' prompt even though you already have an item saved?
In addition, can you take a look at the date created on the bottom of each item and let me know if this gives any clues, is the date the same? Does it tally up with dates you potentially signed in to Amazon?
Any information you can provide surrounding the above questions would be useful.
