I ended up "solving" this issue (or, rather, it stopped occurring) by changing my 1Password password. I'm not sure if this is related but I'm no longer getting the 411. When I was gettin the error, it occurred regardless of whether I typed my password or used the biometric unlock.

Thank you for reaching out to us!
To begin debugging, can you please let us know whether this is happening with the Biometric Unlock enabled, or with the manual authentication flow?
Additionally, if the case is the latter, do you remember if your account has been added via op account add, or by clicking the Add new account button, that appears when running a command unauthenticated?
