I'm sorry that you're running into issues when saving a login using 1Password in the browser. You wrote:
When I create a new account in browser, the extension asks if I want to save the login. I confirm that, and then two identical logins appear in the extension. However, neither of them shows up in the Windows app.
When I click "Edit" on one of those logins in the extension, the app should open with the edit view – but nothing happens.If you lock and unlock the 1Password desktop app, then does the new login item show up there? And when the issue does occur, do you see the item when you log in to your 1Password account on 1Password.com (in the browser, not the app)? Is it duplicated on 1Password.com as well?

I can see the duplicate on 1password.com in the browser.
The login appeared in the desktop app after restarting the computer, and I see it twice there as well. The app itself tells me it's a duplicate.- 1P_Dave

Thank you for the reply. Are you able to reproduce the issue on another website? For example, if you save a login for our test website then do you see two Login items in 1Password:

Hi Simon, yes I believe so. I found the setting "Offer to save and fill logins" confusing because I want to fill, but not to save, and when I disabled "Offer to save and fill" the "Offer to save" option is not available. But I then decided to try saying yes to that but no to "Offer to save items in autofill suggestions" and this seems to be working as I'd hoped.
A doc item on what this autosave feature is trying to do and when it tries to do it, might be helpful to others. From where I sit, autofill is the central feature of 1P, while how the autosave is supposed to work remains mysterious, and thus why the two would be coupled together in one setting is equally so. I see doc on manually saving https://support.1password.com/save-fill-passwords/ but could not find anything in the doc that explains the autosave.

No, it is still happening.
Here are my settings
and here is the unwanted/ not understood pop-up. (Google wasn't used to sign in to EveryAction. 1Password was used to sign in to EveryAction)

In the 1Password extension setting try disabling "Offer to save sign-in-with logins"
