1Password is loading a very old email address into a general web form

I have just produced a detailed step by step listing with screen shots of my issue and I cannot load it because your document does not support PDF or Word documents it seems??? Useless! 1Pa...

1Password uses the email in your Identity item(s) stored in 1Password. Here's an example:

If you look in your Identity items in 1Password, do you have an Identity item with this old email address? Let me know.
