Autofill Not Working for Syniti / Quadrate Website

When I go to the Syniti Data Workbench site (formerly known as Quadrate), the Autofill doesn't fill the form properly. It 'resolves' fine - meaning I can see the pop up in Chrome asking for 1Password...
