Chrome Autofill on Verizon website redirects to "Forgot Password"

I did some testing on my end and I was able to replicate the behaviour you mentioned. It looks like 1Password's auto-submit feature is the culprit here and so I've added you as an affected user to an internal report we have for this issue.
While I can't guarantee a timeline for a fix to be implemented, you can make sure you have access to any new features or fixes as soon as they're available by making sure you have 1Password set to update automatically. In the meantime, you can turn off auto-submit for this specific item with the following steps:
- Click the 1Password icon in your browser's toolbar.
- Search for the login item.
- Click the three vertical dots in the top right of the window and choose Don't sign in automatically.
