You might find our blog post "1Password and 2FA: Is it wrong to store passwords and one-time codes together?" a worthwhile read - it discusses the trade-offs involved in storing TOTP codes alongside passwords.

The step-up authentication idea is definitely worth exploring, and I can see how it could be helpful in scenarios like the one you've shared. I've filed a feature request with the team, including all the of details you've shared here.
