Short answer is that there isn't a custom item option when creating a new item in 1Password. What I would typically recommend for this would be to start with a Secure Note, or the item with the most crossover, then add, remove, and rename fields until it suits the type of information you're trying to save. For your scenario, starting with a Software License, or Wireless Router item could work. For helping to group these items, you could move them to their own vault, or use tags. 

That said, I'd be happy to share your interest in a custom item, or custom template option with the team. Could you share any additional details about the options you'd like to see available with such a feature, or how frequently you might use it?
