Why has Google Chrome suddenly changed 1Passowrd icon to post-it-note from regular 1Password icon ?

This is odd indeed and I can assure you it's not expected behaviour.

Does the icon display persist if you restart your Windows PC once more?

Could you try creating a new Chrome profile and installing the 1Password extension there: Use Chrome with multiple profiles - Computer - Google Chrome Help

Is this icon being displayed in the new Chrome profile? Make sure to not sign in to any Google accounts in the new profile.
