Why were duplicate entries created across ipad and mac?

All entries were entered on the mac. Duplicate entries seemed to be created when I accessed 1password on the ipad. I'm fine with deleting the duplicated entries, but how do I prevent this from happening again?

    Hey @allynschiavone:

    I understand that you're seeing duplicate entries on your iPad. If you sign in to your account on my.1Password.com, do you see these duplicate entries? Do you happen to have a vault named Primary on any of your devices?
