Is all data encrypted?

So anything we enter into 1Password itself is encrypted, expect for the quantity of certain things including the number of vaults, items, and files in our accounts. What exactly is considered a "file" in 1Password? Is that a document (such as a PDF or JPEG) we may put into 1Password?
If I remember correctly with the agilekeychain (I think that's what it was called) format, information such as item titles and usernames were not encrypted. But when the opvault was introduced after the agilekeychain format, everything was encrypted.

That's a blast from the past! The Agile Keychain format was used back in 2008 and was replaced by OPVault in December 2012 (a long time ago). The Agile Keychain kept some information (most notably Title) unencrypted while the current format encrypts all of an item's contents. Metadata (such as creation date, category, etc...) is not encrypted.
