I forgot to mention in my post that I started this process with that setting disabled (as it has been since I upgraded to macOS 15 with 1Password already installed).
As you can see in the below not-at-all-complicated image, for this particular site the passkey creation process prompts me to enable the system AutoFill setting, or provides the option to use a physical security key or scan a QR code to create the passkey using an iOS device. No combination of options triggers 1Password for Safari to open its passkey creation dialog as expected/desired.
This was why I had tried enabling the system AutoFill setting, which then allowed the passkey to be created, but only via the built-in Passwords app. I've since deleted that passkey and re-disabled the system AutoFill as/before you suggested. since it only leads to conflicts with the 1Password Safari extension.
I'll mention that of the 40-ish sites that I added passkeys for, this was the only site that has this issue, so really this seems like a bug report for 1Password for Safari passkey compatibility with the Synology Account site, which https://passkeys.directory/details/synology, rather than a bug report about 1Password's absence from the AutoFill & Passwords Settings pane, as I initially reported.
Does that help clarify the issue I'm having?
In the meantime, I've just reverted to normal 2FA for this website until the issue is resolved, and I suggest that if it cannot be resolved quickly, that Passkeys.directory be updated to reflect the site's incompatibility. Since, as I said, every other site I've tried that's listed as compatible has indeed worked correctly.
