Quick Access Disappearing After Entering Password

After I type in my password to unlock quick access, quick access disappears, sometimes leading me to type in the wrong application when I thought I should be searching for a password. 1Passwor...

I do have "show one password in the menu bar" turned on and this still happens, sometimes it even hides the active application (looks like it hits the keyboard shortcut CMD + H).
