Scanning QR code Fails
I'm trying to active 2FA from Cisco Duo. I think it's an iframe in the browser. I've tried everything in the app to scan the QR code and it doesn't detect it. This isn't the first time. I'm wondering why 1Password doesn't allow me to just upload a QR code? That would solve everything. I've opened this image in its own tab, I've screen shotted it and opened it with Preview. I've directly uploaded the QR code as a file in 1password too.

This might actually not be a 2FA QR code, so maybe that's why it's failing. If I try to capture it on the 1Password app via my phone camera it also doesn't work.

My comment about uploading a QR code still stands though, I've had other times where the QR code detection doesn't work. I'll close this issue though

I'm sorry that you weren't able to scan a QR code. 1Password only supports two-factor authentication that corresponds to the TOTP standard. If the QR code doesn't contain a TOTP secret, but is meant for a different purpose, then 1Password won't be able to scan it.
If you are trying to scan a QR code using the 1Password desktop app and that doesn't work then you can try the browser extension instead: Use 1Password as an authenticator for sites with two-factor authentication
