Selecting "All Accounts" doesn't use logins from all accounts?

Maybe I don't understand how this works. I have "All Accounts" selected, but not all logins show up. I have to manually select a different account in the list each time to get the login information I want.

Is this intended? Or does All Accounts exclude some things?

I have been a paid customer for years, would really like this to be resolved
