Spoke too soon. The extension auto-unlocked a couple of times, but now it no longer unlocks on app-unlock at all, unless you click on the extension icon in the toolbar.

Send a diagnostics report (browser extension)
Attach the diagnostics to an email message addressed to support@1password.com
With your email please include:
