Been having the same issue for some time now, is there any timeframe for a fix ?  This only happens  on safari, In Firefox and Chrome the extension matches the lock status of the app. Searching though the community there are many people having a similar experience.

The issue remains open in our developer's backlog. I've added your report to the internal tracking item for the issue and I'm sorry for any confusion caused by the delayed updating of the 1Password icon in Safari.
