Unique Online Form

How can I use 1Password to fill out a unique online form? Which type passcard would I use? Can I get 1Password to save the exact fields that are in that form?

Hi bobarnett​, thanks for following up with me. I'm sorry to hear it only saved some of your information, and that the extension and Windows app may not be syncing. 

Is this form on a website that's publicly accessible? If so, we'd be happy to take a look to see if there are any better options for setting up an item to fill as expected. 

Also, are you now seeing the identity in 1Password for Windows, or does it appear that it's not syncing as expected?
