Did some testing. The issue with the first clicks occurs one some sites, not on all. Some examples:
Works:

Does not work:

https://acdid.acdsee.com/login
I've chatted with the team about this behavior. The websites that automatically show suggestions from 1Password have coded their login fields with HTML attribute 'autofocus'. The 'autofocus' attribute allows for an element to be focused on page load. This essentially puts a cursor inside the element, which is what triggers the suggestions from 1Password to appear.

The websites that do not automatically show suggestions are not using the 'autofocus' attribute. I'm afraid that we cannot change the code on those websites. However, there is a way to quickly focus on those login fields and to get 1Password's suggestions to appear. When you're on one of those websites, press Tab on your keyboard to focus in on the login fields.
Additionally, when you're on one of those websites, you can invoke the Command+\ shortcut to improve the overall experience. Here's what that does:
1) If you only have one login on the website, this shortcut will fill your login to the fields.
2) If you have multiple logins on the website, this shortcut will open the Quick Access menu which would show all related logins. Click on the login you want in order to fill it.
