Sorry, I forgot to include the versions of 1Password and the Safari extension. I'm using the nightly release of the main 1Password app, currently 8.12.8 (81208019) and I'm using 1Password for Safari Version 8.12.4 (81204046). I had been using the TestFlight version 8.12.6 Build 81206035 until a few days ago, but switched to the public release to see if it was any better. It wasn't!
It happens multiple times per day so I can easily get a diagnostics report. I'll do that asap.
