Hi @16tech89:
Do you have any content restrictions enabled?
Settings > Screen Time > Content & Privacy Restrictions > Content Restrictions > Web Content has to be be set to "Unrestricted Access" or you will be unable to enable the 1Password extension.
If that isn't the issue, let me know and I'd be happy to take a further look at what's going on.
