I have this to offer re saved documents and updating. Is there a better way?

Thanks for your thoughts on this! There's a few possible options you can use, depending on whether or not you're using 1Password 8 or not.

1Password 8 for Mac allows the ability to replace the file in a document item, where 1Password 7 doesn't.

The other possible option is using the Medical Record item type, filling in your details, and then attaching your file to the Medical Record item. With a Medical Record item, you can remove and replace attachments as needed, without having to create an entirely new item every time.
