Missing Items in Sort

With the most recent update for Mac (I'm on 8.12.0, 81200013), "Frequently" or "Recently" used listings are very broken. But its broken on iOS as well.

First: it lists only those that it thinks are "never used". There are no other groupings. 

But even this isn't accurate, as I see items in the list that I know I used within the last week. However, it's there as "never used". 

There are numerous entries missing from this list - most notably, my most frequently used ones. They aren't out of order, they're just not listed.

Can we get this looked at and fixed?

    The behaviour you're seeing when sorting items by "Frequently Used" or "Recently Used" is a known issue, and our developers are currently working on a fix. I've added your report to the list of affected users in our internal work item, and I apologise for any inconvenience caused.
