Which version of 1Password are you using? You mentioned the classic extension so I would assume 1Password 7. Can you confirm? Which OS are you working with? Providing these details during your initial post will help us get you an answer quicker.
It seems like the autofill feature does not work when you have multiple logins for one site AND require a OTP to be filled in.
You have confused me. 1Password can fill multiple logins and OTP. The classic extension is no longer receiving updates. I would encourage you to try 1Password in the browser. You could be encountering and issue there.
